I created this test contact form using php, I tested it on my localhost server and it worked but I’m not receiving the test message using your server! Is there any special configuration I need to do to this form?

What are you using to send the emails? PHP mail() or SMTP?

PHP mail() is severely restricted here, please read this KB article:

If you’re using an SMTP, then please review if your SMTP credentials are correct.
