The short answer: just code it.
A longer answer: I’m not sure what exactly you’re expecting us to do here for you. I don’t know whether you want us to walk you through the entire software development lifecycle, to basically just build the thing for you, or to teach you enough PHP and software development skills that you can do this yourself, But regardless of which it is, that’s a very long process, which is way too big to be addressed in a forum post.
